Library Card Request Form

Piscataway residents who do not have a library card are invited to sign up for one, using the form below.

Your new library card number and PIN will be emailed to you and your card will be mailed to you.   

You may also call Kennedy Library 732-463-1633 x5 and leave a message to receive a call back to complete your application by phone.

IMPORTANT NOTE : Please allow up to two weeks for your card to be issued.
